Permission Form – Youth Programming

To participate in youth programming, a permission form must be on file to attend. Please print and fill it out, or find one at the entrance of the Church.

If you are under the age of 18, please complete the below and return to the Ministry Leader or Rachel McNamara.

Media Waiver

As a part of the Diocese of Hamilton’s Social Media Policy, all young people under the age of 18 will need a Media Waiver filled out and filed with the Youth Minister.
